Insurances and rates:

All of our therapists are considered In-Network with most major insurance companies:

Aetna, Ambetter, Blue Cross/Blue Shield, BCBS FEP, Cigna, Health Advantage, Municipal Health, United Health Care, UMR, Qual Choice & Tricare: Deductibles and Co-pays vary per insurance policy.

EAP Information:

EAP coverage is not always associated with your insurance company and is unusually a separate insurance paneling & billing process. Please let the front office know about EAP coverage up front, providers might not be in network with each individual EAP service.

Self Pay Rate:

Our self pay rate for any hourly service with all of our providers is $100.

Medicaid:

Our practice is not currently a Medicaid/Medicare approved facility, and none of our providers are considered In-Network with these insurance policies.
